Oak Street Health is highly profitable at the location and recently signed a brand new 15-yr lease with 2.50% annual rent escalators.

The Asset is 100% leased to Oak Street Health MSO, LLC (the “Tenant”), a leading provider of primary care for Medicare-eligible adults and the chosen provider of AARP. Oak Street Health was acquired by CVS Health (S&P: BBB) in May 2023, and serves over 225,000 patients in 225 locations. CVS plans to open 26 Oak Street Health clinics inside existing CVS stores in 14 states by the end of 2024. Oak Street Health has reduced patient hospital admissions by 51% and 30-day readmission rates by 42%.
